Work Placement Observations and Witness Testimonies

Introduction

If your online course includes workplace observations, it is essential to complete and submit a Placement Setting Form before beginning your practical tasks.

This ensures your placement tasks are witnessed by an approved workplace assessor, as required by the course awarding body.

Workplace Assessor Eligibility  

Your workplace assessor (or witness) must meet certain eligibility criteria, depending on your course.

This is detailed in your Placement Setting Guidance, which you can find in the Course Introduction section of the LMS (Learning Management System) account.

Completing Your Placement Setting Form

To submit your Placement Setting Form:
  1. Download a blank copy of the Placement Setting Form, from the Course Introduction section of the LMS.
  2. Fill in all fields digitally
  3. Print the completed form, then add all handwritten signatures. Please include all witnesses in the form.
  4. Scan the signed form as one PDF document before uploading it to the LMS.

Form Approval Process

Once we have approved your placement setting form, an email will be sent to the placement email address you provided.

If your form is not approved, you will be notified, and the reason will be shown in the Placement Setting - Submission History section of the LMS.

Idea
If your employer has not received an email, before contacting us, please check if the email you submitted is correct, and ask your employer to check their spam/junk folder.

During this time, your LMS will display your placement setting form status as waiting for employer approval.

You will receive an email notification as soon as your employer approves your placement form, you can then undertake the observations required for your course.

Witness Testimonies 

To evidence you have undertaken your workplace observation, your witness will be provided with witness testimony forms, used to describe the actions you completed during the observation.

Each witness testimony form must include a handwritten signature (not computer-generated), and times and dates of the observation. 

If your Witness Testimony has been referred, the reason will be provided as part of your unit feedback.

If your witness changes during your course, to avoid delays in your work being marked, please submit an updated form with the new witness details (contact us using Chat Support to ask for your placement setting form to be unlocked).

You  must ensure the witness signatures align with those on the placement setting form. Placement forms and witness testimony forms will not be accepted if the signatures are not handwritten and/or do not match
